The ESS-C-133 DRS Counselor Monthly Update Form (Rev. 2021-08-11) will be completed and submitted to the DRS Counselor as per the instructions in each contract. Refer to each contract for specific instructions for each milestone/service.

EC will complete the ESS-C-249 Employee Satisfaction Survey during the Stabilization and Successful Employment Milestones to ensure individual's satisfaction with their employment. The form must be completed before a successful closure. The form will be completed and submitted with the required documentation for payment at the time of completion of the milestones.

The EC will document on the ESS-C-310 Assessment Milestone Report the information obtained during the assessment(s). The EC will include on the form the individual's previous jobs, education level completed, training completed, job skills, their job/career interest, prospective employers and jobs identified, the supports and/or accommodations needed on the job, and the individual's expectations of work. The EC will summarize their observations and add additional comments at the end of the document.
