ESS-C-185 Job Accommodation Form (Rev 2022-09-09)

The EC will complete the Job Accommodation Form when the individual first begins a job, at each milestone, and for any job replacements. The Job Accommodation Form will be submitted for payment with the required documentation for each milestone or service and a copy will be retained in the individual's case file. ESS-C-161 (1) Job Analysis (Rev 2022-09-22)

The Job Analysis must be completed by EC before the individual's first day on the job. Use the Situational Assessments and Job Analysis to complete the Compatability Analysis. The completed ESS-C-161 will be submitted to DRS Counselor by email for payment with other required documentation for payment and a copy retained in the individual’s Case Documentation file. In the Supported Employment Contract, there will be two Job Analyses conducted. If a client loses a job and is replaced, another Job Analysis will be completed before the first day of employment.

ESS-C-338 R4 (4-weeks Employment) Milestone Report (Rev. 2023-08-09)

The ESS-C-338 R4 (4-weeks Employment) Milestone Report will be used in both the Supported Employment (SE) and the Employment and Retention (E&R) contracts. The EC will document the individual's progress and challenges identified during the first 4 weeks of employment. The EC will document their level of performance, and strengths identified by the employer, EC, or individual and describe how they are being reinforced. The EC will also document in detail any areas of job performance that need improvement and describe the steps taken to address the issue(s).
