The EC will obtain a copy of the paystub or earnings statement.
The paystub lists details about the pay rate, hours worked and benefits received. A pay stub is part of a paycheck that lists details about the employee’s pay. It itemizes the wages earned for the pay period and year-to-date payroll. The pay stub also shows taxes and other deductions taken out of an employee’s gross earnings. And, the pay stub shows the amount the employee actually receives (net pay).

The EC will complete the ESS-C-225 (2) Record of Hours SE ST (HC) work for each week the consumer is employed. The documentation will include time spent providing on-site and off-site support and training to the consumer and the time spent making employer contracts. The EC will document specific training/support provided to the consumer. Documentation of support and training will be filled out each week by the EC. The EC will submit completed forms with the required documentation for payment and retain copies in the individual's case file.
